Provide basic case management services to medically fragile children who are at risk of institutionalization and whose health, safety, and well-being can be maintained in his/her primary residence. Coordinate services with RN to ensure quality care.
Required Education
Graduation from an accredited School of Nursing. Ability to direct and carry out prescribed medical treatments and therapeutic programs. Thorough knowledge of professional nursing theory and practice. RN certification with one year of nursing experience preferred.
Required Licensure/Certifications
Valid NC driver's license and a clean driving record (this is a condition of employment) Personal auto liability insurance is mandatory if transporting clients in private/personal vehicle(s). Must meet all other conditions of employment related to criminal record screening, drug screening, reference checking, ect
Essential Physical and Sensory Requirements-With or without the aid of mechanical devices
Regularly required to lift 30 lbs. Must be able to lift a minimum of 70 lbs. Must be able to pull minimum of 50lbs. Must be able to squat, kneel, crawl, crouch, climb, and stoop. Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls. Required to regularly stand and walk. Must be able to demonstrate proficiency in CPR from the floor level requiring to work on hands, knees, bending, standing and lifting. Vision requirements include close vision, distance vision, and peripheral vision. Must be able to talk and hear

Payrate: $30.00-$32.00

Remote work from home primarily, but home visits are required.

This position provide basic case management services to medically fragile children who are at risk of institutionalization and whose health, safety, and well-being can be maintained in his/her primary residence.

Responsibilities include:
  • Establish eligibility for services, obtain client histories, perform initial assessments, develop a plan to provide basic services, explain available services and programs, and refer clients to appropriate programs/agencies for other needed services.
  • Evaluate and document client programs
  • Conduct interviews and work closely with clients, families, other relatives, school, medical providers, and other community agencies contacts to obtain information to be incorporated into a social history which will aid in planning for required services as appropriate to the individual plan of care.
  • Provide basic case management services as the lead member of a Social Worker/RN interdisciplinary team.
Pre-employment screening:
  • Complete criminal background
  • Name checked in the registries. (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities)
  • Drug testing
  • Education verification and other credentialing based on position requirements.
  • Proof of employment history or references (if required)
  • Positions that require driving Proof of driver's license, driver's insurance, and vehicle, IF required for providing transportation for individuals.
